Kindergarten Registration Process Only
- Verify your child's kindergarten eligibility. Your child meets the age requirement to enroll in kindergarten if they are 5 years old on or before August 1 and are a resident of the Willoughby-Eastlake school district.
- Enroll your rising kindergartner using the Online Registration Form (24-25 School Year)
- After June 1, 2024, contact Central Registration.
- Submit all required paperwork online
* Please note: If your child currently attends our W-E preschool program, you do not need to complete the New Student Registration.

Registration Requirements
Completed Online Registration
Original Birth Certificate
Immunization Record
Two Proofs of Residency (See Below).
Custody Papers–finalized with a court stamp
Driver’s License as proof of parental identity
Last report card or transcripts

Deed, Building Permit, Purchase/Rental Agreement, Mortgage Statement,Tax Statement, AND
Current electric or gas bill showing name and address
(We cannot accept a driver’s license, water bill, sewer bill, waste removal bill, cell phone bill, or cable bill.)
Residency Affidavit Notary Form
If parents are living with another family within the Willoughby-Eastlake District boundaries, they must provide a residency affidavit. The form MUST be notarized. In addition, the following items must be submitted: copy of the homeowners mortgage statement, property tax bill, purchase/rental agreement, or the lease agreement, AND a current gas or electric bill of the homeowner.

NOTICE: READ CAREFULLY: Knowingly falsifying this document is a violation of the Ohio Revised Code: Section 2921.13(A) (6) which is a First Degree Misdemeanor punishable by a prison term of six (6) months and/or a fine up to $1000.00. Further the affiant will be billed (and prosecuted in court, if necessary) to collect all back tuition which may be due.
