***You must live in the Willoughby-Eastlake City School District and your child must be potty-trained***
FOLLOW DIRECTIONS AND FILL OUT APPLICATION COMPLETELY
**Please note that two emergency contacts in addition to the parents are required**
The following items are needed to register your child for preschool. All items are due BEFORE your child may start school unless otherwise noted.
______ Copy of Birth Certificate (CAN BE UPLOADED)
______ Updated Immunizations (CAN BE UPLOADED) or faxed to 440-975-3799
______ Updated Physical (NEEDED AT THE START OF SCHOOL)
______ 2 proofs of residency (CAN BE UPLOADED)
- Deed/mortgage/purchase agreement/lease
And one of the following:
- Employment/Payroll Record
- Recent utility bill
- Voter registration
- Income tax record
- Insurance record (home/renter)
- Welfare statement
______ Preschool Parent Questionnaire (ONLINE REGISTRATION)
______ Parent income levels for poverty guidelines (ONLINE REGISTRATION)
______ Custody papers (IF APPLICABLE) (CAN BE UPLOADED)

Please note that the Willoughby-Eastlake Preschool is able to offer ten tuition waiver/reduction placements for the 2023-24 school year through the Early Childhood Entitlement Grant Program. In order to qualify, your child must be 4 years old by 8/1/23 . These placements will be awarded to families with the greatest amount of financial need.
